Total Carnage sends Captain Carnage and Major Mayhem into Kookistan, where General Akhboob is building an army of mutants and holding civilians hostage. The Doomsday Squad's response is a deliberately exaggerated action campaign filled with enemy troops, fortified targets, and screen-filling explosions. You can tackle it alone or share the battlefield with a second player in simultaneous local cooperative play.

The overhead view gives you room to move around incoming fire while aiming in a different direction. The control pad moves your soldier, and the face buttons set the direction of fire, making it possible to retreat while keeping an enemy under attack. Learning how to combine movement and shooting is important when opponents approach from several sides. A second player can join the action, adding another firing position as well as another soldier to protect amid the chaos.

Your regular gun is supplemented by pickups such as grenade and rocket launchers, plasma fire, flamethrowers, and defensive blades. These stronger weapons have limited supplies, so collecting one changes your options for the next encounter rather than permanently replacing the need to aim and dodge. Shields and speed boosts create brief opportunities to push through a dangerous area. Timed explosives add another choice when attacking an emplacement or covering your movement.

The campaign moves through rescue territory, an airfield, and the mutant operation, with hostages and major enemy targets giving the shooting a direction. Crowded fights reward keeping an escape route open, collecting useful supplies, and directing fire toward threats before they surround you. Its cartoon military satire and cooperative structure make the tone boisterous, while the independent movement and aiming controls supply the skill challenge underneath the spectacle.
